The two PDF files below
contain examples of key data elements that should be considered for inclusion in
paper or electronic forms when a patient is transferred from a Hospital to a
Nursing Facility. The first is the current draft of a Hospital to Nursing
Facility transfer form developed by a multi-organizational working group in
Florida led by the Florida Hospital Association. The second is the Massachusetts
Department of Health Universal Transfer Form.
These forms are intended to provide examples of what multi-organizational groups
consider key data elements for information transfer from hospitals to nursing
homes.
